Screenshot or Save the image of your bank transfer, GCash receipt, deposit slip. Please see the image below for reference.

Your Information
Fill-out the form in the right side with your name and your email address. Make sure you choose the correct type of payment (Homecoming, Membership, T-Shirt, Polo Shirt or Donation) and Mode of payment (GCash, PNB or Through Secretariat)

Upload File
Click the Choose File Button and attach your payment details image you used and click the open button in the dialogue box.

Submit
Click Submit Button and wait for the email response “Thank you [your-name] for your payment. It has been recorded”.
